Masters in Healing arts licensed and unlicensed) PURPOSE:
To provide clinical and behavioral services and supports to consumers, assist in development of Preliminary and Comprehensive Rehabilitation Needs Assessments and Individualized Rehabilitation Plans for individuals with developmental disabilities and mental health issues living in the community.
Provide training for consumers and staff to enable these individuals to reach their highest levels of independence in the community.
REQUIREMENTS:
Staff responsible for direct supervision of behavior support plan including the plan's development, training, implementation, and monitoring data collection/accuracy, will at a minimum have:
A Master's degree from an accredited college or university in the mental health discipline.
A licensed mental health worker in the state of NJ:
licensed clinical social worker (LCSW) , Licensed Practical clinician (LPC) or an LSW or LAC working towards independent licensure.
Two years of experience in working in the mental health and/or developmental disabilities and/or addictions fields providing direct care at the master's level; If unlicensed, must actively work towards obtaining first level of licensure within 6 month of employment.
Possess all credentials required by State and Federal Law Driver's license valid in the State of NJ with good driving record Employee must cooperate with the licensee and Department staff in any inspection or investigation.
GENERAL
Responsibilities:
Must successfully complete and demonstrate proficiency in all areas of required training; Assess behavioral/psychiatristneeds, develop and implement appropriate plan of interventions, assess and modify plan in response to changing health care needs of individuals served; Develop informal behavioral support plans for consumers and training for CSS staff; Develop curriculum for staff training necessary to implement the behavioral plans.
Training will include application of the behavior plan, including but not limited to, how to collect data and how to chart/document requirements.
Act as a liaison between consumers, community members, agency and the Department.
Ensure consistent service delivery which connects work, home and recreational needs.
Participate as an active member of a consumers' interdisciplinary team by ensuring completion of the Comprehensive Intake Assessment and Rehabilitation Plan.
Train staff -Orientation to Behavioral Management Techniques and on a consumer's informal behavior support plan.
Provide direct behavioral interventions as needed to provide effective role modeling for staff.
Interact with other staff to ensure continuity and coordination of care.
Effectively use community medical/psychiatric resources in providing care.
Communicate effectively with supervisor, other support staff, clients, families and outside agencies as appropriate.
Participate in client-centered meetings and staff meetings, as appropriate.
Respond to crises and interventions, as needed.
Review behavioral/psychiatric/medical records of individuals served and update information on an ongoing basis.
Recommend policies and procedures on behavioral/psychiatric issues.
On call 24 hours a day in cooperation with management staff.
